Spine Symptom Urgency Check
Leg pain, numbness, weakness — does it fit me, and how urgent is it?

Quick answer (BLUF): A 0–15 score for how dense radiating leg pain, numbness, and weakness are, to sort self-care vs a booked visit vs a spine surgeon within days. Not an MRI reading and not an order to operate. Saddle numbness or loss of bladder control: call emergency services now.
5 core items (0–3) + 5 profile + 5 urgent items (not in the 15) — not a surgical order
How to answer
Symptoms: 0/1/2/3 · Other: Yes/No. Any urgent Yes → call emergency services (Thailand 1669) now, even if the score is low.

Part 1 — Core symptoms (scored)

Q1: Pain radiating down the leg
In the past 2–4 weeks, how often did pain travel from the back into the buttock, calf, or foot?

Q2: Numbness in the leg or foot
How is numbness along the leg or in the foot?

Q3: Weakness / foot drop
How is leg or ankle strength?

Q4: Sitting, walking, sleep
How much do symptoms limit sitting, walking, or sleep?

Q5: Time course after rest and physiotherapy

Part 2 — Profile (not in the 15)

1. Heavy lifting, repeated bending, or long sitting without a position change is part of the job

2. Prior spine surgery, osteoporosis, or a history of cancer

3. Daily anti-inflammatory tablets, or a blood-thinning medicine

4. Deciding on surgery from a group MRI photo, without the radiology report

1. Numbness of the buttocks, genitals, or around the anus

2. New inability to hold or pass urine or stool

What a 0–15 score means

Spine-symptom urgency bands — not a surgical order and not a film reading

ScoreBandMeaningWhat not to do
0–4Green — self-careMove as tolerated and note when pain radiates. A group photo is not a radiology reportDo not skip urgent items — a low score does not rule out a disc
5–7Yellow — book a visitSee orthopedics or a spine surgeon and bring the written report, not a screenshotDo not decide on surgery from comments
8–10Orange — spine surgeon within daysEspecially if sitting, walking, or sleep is barely possible, or there is weaknessDo not stack extra pain tablets on your own
11–15Red — same-day evaluationIf any urgent item is Yes, call emergency services (Thailand 1669)Do not drive yourself if a leg is weak or the saddle area is numb

Frequently asked questions

Does this score say whether the disc should be operated on?

No. It measures symptom density and urgency. Surgery is a shared decision with the surgeon who examined you and read the report.

I have saddle numbness or cannot pass urine. Should I finish the check first?

No. Call emergency services now. Do not wait for a score or for group comments.

Can a group MRI photo replace the report?

No. The level and the degree of compression come from the radiology report and the strength exam.

My total is low, but the foot will not lift. What does that mean?

New weakness is not cancelled by a low total. See a spine surgeon promptly. Do not wait out a week-count from a post.

Do many people improve without surgery?

Yes, in trials. Leg pain often eases faster with earlier surgery, while later results can be similar. This score does not choose the path.

How is this different from the heart score?

This layer is radiating pain, numbness, and weakness from the spine. If palpitations or chest pressure dominate, use the heart symptom check.
