Alopecia Areata in Asian Research: What Studies Show, and What JAK Inhibitors Do Not Promise

Short answer

Alopecia areata has Asian studies and large drug trials. The leading model is immune-privilege collapse at the growing follicle, via CD8 NKG2D cells, interferon-gamma, and IL-15. In 300 Asian patients, dermoscopy found yellow dots, black dots, and tapering hairs. Oral JAK inhibitors help some people with severe loss, not everyone.

1. There are Asian studies

A Global Burden of Disease 2023 analysis of China, Japan, and South Korea found higher age-standardized incidence and prevalence in Japan and South Korea than in China and the global figure for 2023. Women carried more of the burden. Incidence clustered in young and middle-aged adults. The same analysis found ecological correlations with atopic dermatitis, iron deficiency, and thyroid disorders. A country-level correlation is not a diagnosis for one person (PMID 42808033).

The dermoscopy study that matches the question about Asian patients examined 300 people. It found black dots, tapering hairs, broken hairs, yellow dots, and clustered short vellus hairs. Yellow dots and short vellus hairs were the most sensitive markers. Black dots, tapering hairs, and broken hairs were the most specific (PMID 18613874).

The large drug trials are multinational. The abstracts cited here do not report a Thai-only response rate.

2. The mechanism these papers use

Growing follicles keep a zone of immune privilege from the bulge down to the bulb. That privilege is relative. It includes low MHC class I and II and local inhibitory signals. In this model, collapse of privilege at the bulb is a step into alopecia areata, and affected follicles are infiltrated by NKG2D-positive T cells and natural killer cells (PMID 32682334).

In mice, CD8-positive NKG2D-positive T cells were necessary and sufficient to induce the disease model. Interferon-gamma and IL-15-family signals were part of the signature. Blocking interferon-gamma or the IL-15 receptor reduced disease in the model, and JAK inhibitors interrupted the interferon signature in mice. That is a mechanism, not a regrowth percentage in people (PMID 25129481).

The strongest genetic signal in a large genome-wide meta-analysis sits in the HLA region. That study was not an Asian-only cohort, so one allele should not be treated as the explanation for every Thai patient (PMID 25608926).

The hair-loss score sorts symptom density. It does not diagnose alopecia areata and it does not start a JAK inhibitor.

3. What changes when the loss is extensive

The numbers below come from trials in severe loss. They are not a promise for a small patch, and they are not an Asian-only rate.

SituationWhat the studies sayWhat they do not say
Limited patchesA dermatologist examines the whole scalp. Intralesional triamcinolone or a topical corticosteroid may be considered. Topical minoxidil can support regrowth. It does not stop the immune attack. The baricitinib and ritlecitinib trials were not done in a single coin-sized patch. Do not inject corticosteroid yourself.
At least half the scalp, adultsBaricitinib inhibits JAK1 and JAK2. At 36 weeks, a SALT score of 20 or less occurred in 38.8 percent on 4 mg and 22.8 percent on 2 mg, versus 6.2 percent on placebo in the first trial, and 35.9 percent, 19.4 percent, versus 3.3 percent in the second (PMID 35334197).Most people on 4 mg had not reached that bar at 36 weeks. Acne, muscle-enzyme elevation, and cholesterol changes were more common than on placebo.
At least half the scalp, age 12 and olderRitlecitinib inhibits JAK3 and TEC-family kinases. At 24 weeks, 50 mg reached a SALT score of 20 or less in 29 of 124 people (23 percent), versus 2 of 130 on placebo (2 percent). Asian participants were 186 of 718 randomized people (26 percent) (PMID 37062298).The 23 percent is the whole-trial result, not a Thai subgroup. Enrollment of Asian participants is not the same as every dose being licensed in every country.
Extensive disease in some clinicsDiphenylcyclopropenone is used to induce a controlled contact reaction on the scalp. It is a different method from an oral JAK inhibitor.Do not buy this chemical for home use. The skin reaction is part of the method and the concentration needs supervision.

4. A smooth round patch is not every kind of hair loss

This table separates patterns. It does not let a group photo make the diagnosis.

PatternWhat is seenWhat it is not
Alopecia areataA smooth round patch with a fairly sharp edge. Eyebrows or eyelashes may fall in patches. Not automatically a fungus, and not the gradual thinning of the crown.
Telogen effluviumShedding across the scalp, thinner hair, no smooth round patch, often weeks to months after illness, birth, stress, or surgery.Diffuse shedding after illness is a different pattern. It is not diagnosed from a coin-sized patch.
Scalp fungusBroken hairs, scale, and sometimes pain or pus, especially in a child. Black dots alone do not separate the diseases. This needs an exam, and sometimes a culture. Do not seal it with a herbal paste.
Pattern thinningGradual loss at the crown or hairline, not a circle that appears over a few days.A JAK inhibitor studied for alopecia areata is not a substitute treatment for that pattern.

5. What dermoscopy can and cannot say

A review found yellow dots reported in 6 to 100 percent of patients, short vellus hairs in 34 to 100 percent, black dots in 0 to 84 percent, and exclamation-mark hairs in 12 to 71 percent. Ranges that wide mean one sign is not a diagnosis. Several findings have to sit together (PMID 29569271).

A chat photo has neither the magnification nor the lighting of a clinic, so it does not replace a dermatologist’s exam.

Signs to have seen soon

  • Patches spreading over days to weeks toward most of the scalp, or loss of eyebrows, eyelashes, or body hair. See a dermatologist soon. Do not wait out a product course.
  • A painful, pustular, or thickly scaly scalp, or a child with broken hairs. Fungus and other inflammation are in the differential.
  • Small nail pits can travel with alopecia areata and should be mentioned to the dermatologist. Pits alone are not a reason to call an ambulance.
  • Full-body hives, swelling of the mouth or eyes, or trouble breathing need emergency care. That is a different problem. Read itchy rash with hair shedding.

Questions people ask

Are there alopecia areata studies in Asian people?

Yes. A dermoscopy study examined 300 Asian patients, and a 2023 burden analysis covers China, Japan, and South Korea. The large JAK-inhibitor trials are international. Some enrolled Asian participants, but the response rates cited here are for the whole trial, not a Thai-only percentage.

Do JAK inhibitors regrow hair in everyone?

No. In adults with at least half the scalp involved, baricitinib 4 mg reached a SALT score of 20 or less at 36 weeks in about 36 to 39 percent, versus about 3 to 6 percent on placebo. Ritlecitinib 50 mg did so at 24 weeks in 23 percent, versus 2 percent on placebo. Many people do not reach that bar, and these are prescription drugs.

What does scalp dermoscopy show in alopecia areata?

In 300 Asian patients the findings were black dots, tapering hairs, broken hairs, yellow dots, and clustered short vellus hairs. Yellow dots and short vellus hairs were the most sensitive. Black dots, tapering hairs, and broken hairs were the most specific. No single finding replaces an exam of the whole scalp.

Is a small patch treated like loss of half the scalp?

No. Limited patches are a dermatology exam, and intralesional corticosteroid may be considered. The baricitinib and ritlecitinib trials enrolled people with at least 50 percent scalp loss. They do not establish those oral drugs for a single coin-sized patch.

Is stress or thyroid disease the only cause?

No. An East Asian burden analysis found ecological associations with atopic dermatitis, iron deficiency, and thyroid disorders. Those are population-level correlations, not a cause assigned to one person. Thyroid-related shedding is discussed in thyroid disease and telogen effluvium.

When should a dermatologist see this soon?

When patches spread quickly, eyebrows or eyelashes fall out, the scalp is painful, pustular, or thickly scaly, or a child has broken hairs. Do not wait out a product course. Full-body hives, mouth swelling, or trouble breathing need emergency care. That is a different problem from alopecia areata.

Mechanism, as assembled by the author

Asst. Prof. Dr. Norawit Raatpiboon assembles the papers below as follows. The current model treats alopecia areata as collapse of immune privilege at the bulb, followed by CD8 cells that display NKG2D and by interferon-gamma and IL-15 signals that run through JAK enzymes. The oral JAK-inhibitor trials were built for severe loss on that pathway. The model gives a direction. It does not promise that any one person’s hair returns to its previous thickness or color.
