A Lung Nodule After Endometrial Cancer: Second Primary or Metastasis, Food, and Recovery After Surgery

Short answer

A lung nodule after endometrial cancer may be a new cancer or a metastasis. Posts cannot separate them. Cutting sugar or timed eating is unproven as cancer treatment. During treatment, diet evidence is thin. Walking helps some people after lung surgery. Regrown hair is not proof of remission.

Where this question came from

One member described treatment for endometrial cancer, later surgery for a lung nodule, and an earlier episode of fluid in the lungs with a heart problem. After eating fewer sweet foods and starches and leaving a long gap between meals, her blood pressure, lipids, and glucose sat in the range her own team uses. This page answers the physiology questions that follow. One person’s course is not a trial result, and the page does not identify her.

Going to the hospital alone, and how visit intervals are set, is a different page: solo cancer survivorship and follow-up.

1. A second tumor is not automatically a new primary

When more than one tumor is found, in the same organ or in another, multiple primary cancers may be present. A review reports frequencies from 2 to 17 percent, because the SEER definition and the IACR/IARC definition are not the same. That range is how often multiple primaries are reported. It is not a survival rate (PMID 28761745).

A 25-year Siriraj series of 109,054 adults with a first solid cancer found multiple primary cancers in 1,785 people, 1.63 percent. The study called a second cancer metachronous when the gap was longer than 2 months. Uterine and ovarian cancers had a strong synchronous association, relative risk 27.77, and the median time from a first uterine cancer to a second cancer was 55 days. Those figures do not say that a lung nodule many years later is the common pattern in that data set (PMID 34556087).

One case report describes a lung nodule nearly 20 years after hysterectomy for endometrial adenocarcinoma. The team treated it as another primary lung cancer. Final pathology was endometrial adenocarcinoma. One case is not a rate. It does show that an old endometrial cancer stays on the list when a lung nodule appears (PMID 33575026).

This table separates a registry label from a laboratory answer. A post cannot assign either label to a named person.

QuestionTwo primary cancersMetastasisWhat separates them
Time labelSome registries say metachronous when the gap exceeds a cutoff, such as 2 months in the Siriraj series.A metastasis does not become a new primary because time has passed.A calendar rule is not a biopsy.
TissueDifferent origins. Treatment depends on the type and stage of each tumor.Tissue from the first cancer growing in the lung, even years later.Pathology compared with the earlier specimen. Some cases need immune stains or molecular comparison.
What a post can sayThat there were two episodes of treatment. That a lung nodule followed an endometrial cancer.Neither the type of the nodule nor the true stage.

2. Cutting sugar and timed eating are not proven cancer treatment

Many cancer cells favor glycolysis even when oxygen is present, the pattern called the Warburg effect. Sugar on a plate and glucose in the blood are not the same switch. The body defends blood glucose. Removing sweets from a meal does not zero the fuel a tumor sees.

A review of intermittent fasting found conflicting rodent results, including possible harm in some settings. Effects on human cancer incidence and prognosis are unknown because high-quality randomized trials are lacking. Early studies of prolonged fasting exist, but the authors would not recommend intermittent fasting during active cancer treatment outside a trial. Fasting may be discussed for weight management in adults interested in prevention, and even then it is unknown whether the fasting itself changes cancer pathways (PMID 34383300).

The ASCO guideline for adults during treatment recommends aerobic and resistance exercise when treatment has curative intent, and says clinicians may recommend preoperative exercise before lung surgery. Neutropenic diets did not reduce infection. Evidence for other diets and for weight-loss interventions during treatment was very limited (PMID 35576506).

The American Cancer Society survivorship guideline notes that laboratory and observational studies suggest diet, physical activity, and obesity may relate to recurrence and survival. That sentence is not the claim that one person’s sugar cut treated two cancers. The same guideline cites a 68 percent 5-year relative survival for all cancers combined in the United States. That blended figure is not the outlook for endometrial cancer plus a lung nodule (PMID 35294043).

If blood pressure, lipids, and glucose moved into range after a diet change, that is a metabolic observation for the heart and cancer teams to recheck. It is not evidence that the diet treated the cancers.

3. What can be done, and what the evidence does not give

Each row separates a reported result from an explanation that goes past the evidence.

TopicWhat the evidence saysWhat it does not sayCaution
Type of the second tumorA time rule in a registry and a tissue diagnosis are different layers. A reported lung nodule thought to be a new lung cancer was endometrial tissue.Successful surgery does not mean every such nodule is early lung cancer, and it does not supply a 5-year survival for every person in this situation.Ask how the pathology of that nodule compares with the earlier cancer. Do not take the word new primary from a post.
Food and eating windowsDuring treatment, evidence for special diets and weight loss is very limited. A review’s authors would not recommend intermittent fasting outside a trial during active treatment. It does not say that fewer sweets cannot help blood pressure, lipids, or glucose. It also does not say sweets are a switch that brings cancer back.Unintended weight loss needs a clinician. Do not cover that symptom with a fast.
After lung surgeryASCO may recommend exercise before lung surgery, and recommends aerobic plus resistance work during curative-intent treatment. In 125 people who had video-assisted surgery and risk-based rehabilitation, stay and complications did not differ significantly across risk groups. It does not prove that the remaining lung grows back the removed part. A breathing device is not the treatment for breathlessness months later. The program belongs to that person’s thoracic team and physiotherapist, not a minute count copied from a post.
Hair after chemotherapyIncomplete regrowth at 6 months or later is defined as persistent chemotherapy-induced alopecia. Most papers in the review are case reports, often in breast cancer.New hair does not confirm remission and does not measure immune recovery.Color and texture can change. If hair is still incomplete after six months, a dermatologist should look. More fasting is not the next step.
Follow-up visitsStage, histology, and the treatment already given set the schedules of the gynecologic and thoracic teams. Those schedules may differ. This page does not assign one CT or MRI interval to everyone.Feeling well does not cancel a scan the team still needs.

4. Breathlessness after lung resection, and rehab around surgery

Removing a lobe or a wedge lowers the volume available for breathing. Breathlessness on a long walk can follow surgery. It does not automatically mean the cancer has returned, and new breathlessness should still be reported.

The ASCO review found that preoperative exercise before lung surgery was associated with a shorter hospital stay and fewer complications. Clinicians may recommend that exercise, and should recommend aerobic and resistance exercise during treatment given with curative intent (PMID 35576506).

A retrospective series of 125 people undergoing video-assisted surgery for lung cancer assigned heart-and-lung rehabilitation from preoperative exercise-test risk. After the program, risk groups showed no significant difference in intensive-care stay, hospital stay, time with a breathing tube, chest-drain time, or postoperative complications. The p values ranged from 0.061 to 0.910. The authors encourage this program around video-assisted surgery. The study was not randomized, and it did not measure regrowth of the remaining lung (PMID 35783624).

A device that rewards a slow deep breath is something some teams use around the operation so the remaining lung opens and is less likely to collapse. It does not regrow removed lung, and it is not the answer to breathlessness at rest months later. Walking pace and breathing practice follow the physiotherapist who knows how much lung was removed.

5. Hair after chemotherapy is not a cancer test

Some chemotherapy drugs shed hair because they disturb the growing follicle. Hair that returns after the drugs stop is common to see. It does not mean every person gets the previous color or texture, and it does not mean the cancer is gone.

A scoping review of 39 papers defines persistent chemotherapy-induced alopecia as incomplete growth at 6 months or later after chemotherapy ends. Twelve papers were case reports, and only two were randomized trials. Twenty-seven were in breast cancer. Alkylating agents, taxanes, and anthracyclines were the drug classes mentioned most often. All six papers that looked at quality of life found effects on psychological, functional, and social life. That record shows incomplete regrowth is a real problem. It is not proof that stem cells throughout the body have recovered (PMID 42888908).

In the same review, scalp cooling was the prevention method with supporting studies during treatment. Reports of topical minoxidil and low-dose oral minoxidil concern management after regrowth stays incomplete. Most of those papers are not randomized trials, so this page does not start either drug. If hair is still incomplete after six months, a dermatologist should examine the scalp.

6. Follow-up belongs to the treating teams

Endometrial cancer and lung cancer do not share one follow-up calendar. Stage, histology, and whether the second nodule is the original cancer or a new one change both the interval and the test. This page has no CT or MRI schedule for every reader.

Feeling well is information the team can use. It does not cancel a pending visit, and it does not replace a radiologist’s reading. Going to appointments alone is covered in solo cancer survivorship and follow-up.

7. People who have had fluid in the lungs from the heart

A history of heart failure changes how urgent new breathlessness is. Being unable to lie flat, coughing frothy or bloody sputum, and sudden chest pain are not symptoms to discuss on a post first. Contact emergency services.

A long eating gap lowers both energy and fluid. Someone with prior heart failure already has a fluid limit set by the cardiology team. Starting a 16-hour gap alone can work against that limit. If blood pressure, lipids, or glucose look better after a plate change, the same teams should check whether the medicines and targets still fit. Do not stop a heart medicine because a short run of numbers looks better.

Symptoms to have seen promptly

  • Breathlessness while sitting, inability to lie flat, or frothy or bloody sputum, especially after pulmonary edema from the heart. Contact emergency services. Do not wait for the next oncology date.
  • Sudden chest pain or new pressure spreading across the chest is also a reason to contact emergency services.
  • Fever with rigors close to chemotherapy is not a problem to solve by cutting sugar.
  • Weight loss that was not planned, trouble swallowing, or a steadily falling intake should be told to the cancer team. Do not score it as success of an eating window.
  • New weakness in both legs, new confusion, or a seizure is different from breathlessness after a walk and needs same-day care.

Questions people ask

How is a second primary cancer different from a metastasis?

Metachronous in a cancer registry is a time rule, not a pathology result. A Siriraj series used a gap longer than 2 months. A lung nodule years after endometrial cancer may be a new lung cancer or endometrial tissue that has spread. In one reported patient the surgical team thought the nodule was a new lung cancer, and the final pathology was endometrial adenocarcinoma. A social post cannot separate the two.

Do cutting sugar and a 16-hour eating window control cancer?

That is not proven in people. A review found that the effects of intermittent fasting on human cancer incidence and prognosis are unknown because high-quality randomized trials are lacking, and the authors would not recommend it during active treatment outside a trial. The ASCO guideline says evidence for other diets and for weight loss during treatment is very limited. Better blood pressure, lipids, or glucose in one person is a separate question from cancer treatment.

What can rehabilitation after lung surgery do, and what can it not promise?

ASCO recommends aerobic and resistance exercise during treatment with curative intent, and clinicians may recommend preoperative exercise before lung surgery because the reviewed evidence found shorter stays and fewer complications. A retrospective study of 125 people who had video-assisted lung surgery and risk-based rehabilitation found no significant difference in stay or complications across risk groups. That does not mean the remaining lung grows back the removed part, and it does not replace oncology follow-up.

Does hair that returns after chemotherapy mean the cancer is gone?

No. A scoping review defines persistent chemotherapy-induced alopecia as incomplete hair growth at 6 months or later after chemotherapy ends. Regrowth is not a cancer test and it is not a measure of immune recovery.

Which symptoms need a hospital the same day?

Coughing blood, breathlessness while sitting or an inability to lie flat, sudden chest pain, fever with rigors near chemotherapy, and weight loss that was not intended. Sudden breathlessness or chest pain needs emergency services, not the next routine oncology visit.

Should someone with prior heart failure start a long eating window alone?

No. Prior heart failure comes with limits on food and fluid. A long fasting window can reduce intake and fluid and can worsen that condition. A diet change belongs with the heart and cancer teams. Unintentional weight loss is a reason to be seen, not a goal of fasting.

Mechanism, as assembled by the author

assembles the papers below as follows. A second tumor is separated by tissue, not by the gap on a calendar. Even when cancer cells rely more on glycolysis than other cells do, defended blood glucose means cutting sugar on the plate is not the same as cutting a tumor’s fuel in a person. Human studies have not shown that an eating window changes cancer prognosis. Lost lung volume can explain breathlessness on a walk, but the rehabilitation data are about exercise around surgery, not regrowth of the removed lung. Hair reflects how growing follicles respond to drugs. It is not an index of the cancer or of whole-body immunity.

This page is general education. It is not a reading of a biopsy, not a diet order, and not a statement that the reader’s nodule is a new primary or a metastasis. Starting or stopping a heart medicine, chemotherapy, or a scan plan belongs to the team that examined that person. Sudden breathlessness, chest pain, or coughing blood needs local emergency services.
